carousel5Community Opportunities, Inc. provides community-based services to individuals with mental health disabilities, brain injuries, mental retardation and/or developmental disabilities. Based in Somerset, Kentucky, supports are available to persons in the Supports for Community Living, Michelle P, Acquired Brain Injury, ABI Long-Term Care Medicaid waivers. In addition, Community Opportunities also provides contracted services to adolescents with mental retardation and developmental disabilities through the Department of Community Based Services.
